Florida Medicaid’s Covered Services and HCBS Waivers
School-Based Services Programs – County Health Department (CHD) Program
Medicaid reimburses county health departments (CHDs) that provide services in public schools for the following:
- Nursing.
- Group social work services
- Individual social work
This service is not one of the minimum covered services for managed care plans serving Medicaid enrollees.
Eligibility
Florida Medicaid recipients requiring medically necessary CHD services must meet the following criteria:
- Be enrolled in a public school; and
- Be under age 21.
