Florida Medicaid’s Covered Services and HCBS Waivers
Child Health Targeted Case Management
Medicaid child health targeted case management services provide case management to recipients who are receiving services from the Children’s Medical Services Early Steps Program, or are in medical foster care to assist them in gaining access to medical, social, educational, and other services as required.
This service is not one of the minimum covered services for all Managed Medical Assistance plans serving Medicaid enrollees.
Eligibility
All Medicaid recipients who meet the following criteria may receive Child Health Services Targeted Case Management services.
- Three years of age or younger and receiving services from the Children’s Medical Services Early Steps Program, or twenty years of age or younger and receiving services through a Children’s Medical Services Medical Foster Care contractor.
- Not receiving case management services under a home and community-based service waiver.
- Not in a hospital, nursing home, intermediate care facility or institution for mental diseases in accordance with coverage and limitations requirements.
