Health Care Facility Updates

The Agency requires all licensees providing residential or inpatient services to use the Health Facility Reporting System (HFRS) to report its emergency status, planning or operations.

Medicaid Updates

Guidance for Provisional Enrollment

For providers within the State of Florida, along with out-of-state providers, who are not enrolled with Florida Medicaid and who provided services to our displaced Florida residents, Florida Medicaid created a provisional provider enrollment process. Florida Medicaid has waived some initial requirements for providers to submit normally required information and documentation when applying for provisional enrollment.

Managed care provider provisional enrollment plans should:
  1. Complete a SMMC Provisional Out-of-Network Provider Enrollment Form[ application/pdf ] for each Non-Medicaid provider providing services to eligible Florida Medicaid Recipients;
  2. Complete a SMMC Provisional - Services Provided Attestation Form[ application/pdf ] for each provider providing services to eligible Florida Medicaid Recipients. Complete the form for the first eligible Medicaid Recipient where services were provided;
  3. E-mail the two (2) forms to Florida Medicaid’s fiscal agent, Gainwell Technologies, at [email protected]. One (1) provider’s information per e-mail; and
  4. For the e-mail subject, enter SMMC Provisional Provider Enrollment – (Health Plan ID & Provider Name).

Gainwell will add the provider to the Florida Medicaid Management Information System (FMMIS), generate a Medicaid Provider Id, and add the Provider Information to the Provider Master List (PML).

Individual providers requesting provisional enrollment for Fee-for-Service claims should:
  1. Complete a Fee-For-Service Provisional Provider Enrollment Form[ application/pdf ];
  2. Complete a Fee-for-Service - Services Provided Attestation Form[ application/pdf ] for the first eligible Florida Medicaid Recipient where services were provided;
  3. Sign the applicable Florida Medicaid Provider Agreement (MPA);
  4. Submit a copy of the provider’s professional license;
  5. Sign a Florida Medicaid Electronic Funds Transfer (EFT) Authorization Agreement[ application/pdf ], if the provider will be then pay-to-provider;
  6. E-mail all the forms to Florida Medicaid’s fiscal agent, Gainwell Technologies, at [email protected]; and
  7. For the e-mail subject, enter Fee-for-Service Provisional Provider Enrollment – (Provider’s Name).

This temporary enrollment process is designed to facilitate the Managed Care encounter and fee-for-service claims payment process for those providers caring for the needs of our Medicaid recipients who were impacted by the recent disaster.
