Online Reporting System - Health Facility Reporting System (HFRS)

The Agency requires all licensees providing residential or inpatient services to use the Health Facility Reporting System to report its emergency status, planning or operations per 408.821(4), Florida Statute.

To obtain an account, contact your facility’s Administrator, CEO, designated HFRS Superuser or Agency licensing unit.
