Expanded Benefits - Equipment and Supplies
Assisted Living Facility (ALF) Move-in Basket – This is a one-time benefit. Members currently living in an ALF and new members transitioning/moving into an ALF can select up to $50 worth of items.
Biometric Equipment – Available to all members with prior authorization. Members may receive one (1) digital blood pressure cuff and one (1) weight scale every three (3) years.
Cellular Services – For all members who have limited or no cell service. Members may receive one (1) cellphone; three hundred fifty (350) minutes; unlimited text messages; and sixteen (16) GB data.
Cooking at Home Catalog – Available to all members with prior authorization. Members may select one item from a supplied list of kitchen equipment including an air fryer, toaster, pot and pan set, blender, and more.
Dental Desensitization Kits – Available to all members annually.
Durable Medical Equipment/Supplies – Members may receive one box fan per year.
Health Grocery Card for Caregivers – Members may receive one (1) $100 Healthy Card per year with prior authorization.
Healthy Living Benefit – This is a one-time benefit. Members may choose any two (2) items from a list of Healthy Lifestyle aids including a wide variety of assistive devices and adaptive aids to help members maintain independence in their homes. Available items include a digital scale, home blood pressure monitor and cuff, peak flow meter, reachers/grabbers, lumbar pillow, personal fan, clip on lamp, walker bag, and a pair (2) of face masks.
Hygiene and Wound Care Kit – Members may receive wound cleansers and hygienic items or devices as medically necessary with prior authorization.
Portable Fall Prevention Supports – Available to all members as medically necessary with prior authorization. Items include a grab-bar with medical grade suction cups, non-skid bathmat and tub liner, and shower chair as well as information on fall prevention.
